Putnam County R-I Schools is excited to once again partner with our amazing local healthcare providers to offer FREE sports and activities physicals for students preparing for the 2026–2027 school year.📅 Date: Wednesday, May 20, 2026
⏰ Time: 9:00–11:00 AM
📍 Location: PCHS Gymnasium
This opportunity is available for current 5th–11th grade students planning to participate in any of the following activities next school year:
• Band
• Baseball
• Basketball
• Cheerleading
• Color Guard
• Cross Country
• Football
• Golf
• Softball
• Track & Field
📄 MSHSAA Preparticipation Documentation
All students participating in MSHSAA-sponsored activities are required to complete annual preparticipation paperwork and physical requirements before participation.
⚠️ IMPORTANT:
ALL required paperwork must be:
✔️ Completely filled out
✔️ Signed by a parent/guardian
✔️ Brought with the student on physical day
